OBLIGATIONS FOR APPLICANTS:
1. Please advise the Foundation of any change of address while enrolled in the program and upon completion of the program.
2. The applicant is responsible to ensure that the completed application and all supporting documents are submitted to the Registered Psychiatric Nurses Foundation by the 15th January.
3. Please ensure that you retain a copy of all your submitted information. Use the print button on page 4 of the application to retain your details.
4. Your completed application will be kept on file by the Registered Psychiatric Nurses Foundation for a minimum period of six years.

6. The financial scholarships will be made directly to the successful applicant and a T4A will be issued prior to March 1st of the following year for the Canada Revenue Agency.
